The Safety Incentive Program
On July 1st, 2005, a new safety incentive program was initiated within the operations and maintenance crews.
Two incentives are now offered that encourage employees to work safely. The first is earned by each shop or cluster individually and is awarded quarterly. The second is earned by the entire Facilities organization and is awarded annually.
To be eligible for the individual shop/cluster award, an employee must be employed during all three months of the quarter and still be employed when the incentive is given. To be eligible for the Facilities-wide award, an employee must contribute to achieving the goal and be employed when the incentive is given.
Each crew will be kept up to date on its progress towards gaining the incentives. Each month, a "scorecard" will be posted in each crew's work area, indicating the individual crew's quarterly score and Facilities' progress toward the annual goal.
Crew Quarterly Incentive Program
5 "areas" of safety have been established, and each is worth 10 points each. A crew will earn points by displaying safe behavior in each area. Therefore, 50 points are available to be earned by each crew per month, resulting in a possible total of 150 points for the quarter. In addition, there is a bonus area worth 5 points.
A crew must earn 90 percent of the total available points -- or 135 points -- during the quarter to gain the incentive.
The 5 areas and the bonus area are described in detail below outlining what is required to earn points:
Monthly Shop Safety Meetings
Shop safety meetings provide a forum for crew-specific safety issues and procedures to be discussed.
- The entire crew will receive the month's training and information discussed during the monthly meeting.
- A sign-in sheet will be made containing the following information: a summary of the training; the employee's signature, expressing understanding of the general topic; the date the training was delivered; safety concerns and suggestions produced from the meeting; and any other pertinent safety information.
- Supervisors have all month to pass on the information to the crew. Every employee must get the safety meeting information to obtain the 10 points. (9 out of 10 employees will merit 0 points!)

Scheduled Required Training
Safety training informs employees on specific regulations and the safety procedures to follow while employed at USU.
- There are sets of specific required trainings for each shop, known as Core Trainings. The supervisor is accountable for communicating to the crew what trainings are required and when the scheduling trainings are available.
- Required training for each crew will be scheduled during specific months throughout the year. The trainings will be advertised far in advance to allow for planning and scheduling. There will be a varied schedule of training to provide all employees with a chance to attend the core trainings.
- 100% attendance is required for the shop to receive the full 10 points. 75-99% attendance will earn 5 points, and less than 75% will earn 0 points.

Accidents
All workplace accidents and injuries can be prevented.
- 0 WCF accidents in a month will earn a crew the full 10 points.
- If an injury occurs during the month that generates WCF costs, then no points will be given for that month.
Accident Reporting Procedure
This option is available to discourage the nonreporting of accidents.
- Points are awarded only if an injury occurs during the month.
- The crew can earn back half of the points lost to the accident if all paperwork is documented and submitted on time.

Safety Training Observation Program (STOP) Audits
The STOP Safety Cycle is used to build overall safety awareness.
- A STOP Audit session is a time specifically dedicated and scheduled for a supervisor to observe the safety behaviors of employees.
- An employee should be scheduled to accompany the supervisor on at least 1 of the scheduled STOP Audit sessions.
- Supervisors are accountable for 1 STOP Audit session a week and 5 STOP Audit sessions a month. Points will be given per session performed. There are 2 points available for each STOP Audit session, with a maximum of 10 points per month.

Manager's Safety Perspective
This category allows for the evaluation of the safety attitude of the crew.
- This point system is derived from the Safety Manager's personal evaluation of the crew's safety culture observed during the month.
- 1 to 10 points may be awarded on a variety of subjects, depending on the manager's safety performance evaluation.
- The criteria may include some of the following items: cleanliness and orderliness; policy/regulation compliance; the understanding and following of procedures; the safety attitude of the crew; and specific goals identified by the manager.
- During a one-on-one session, the Safety Manager will explain to the supervisor the specific reasons behind the score. An understanding should be reached on what contributed to the overall score, both positive and negative.

Safety Suggestions
This category awards employees for being proactive in their safety efforts.
- A crew member who makes a safety suggestion that is afterwards implemented will earn 5 points for his/her crew.
- Bonus points may only be earned once per month.
- Suggestions will be evaluated and implemented through the Safety Committee.

Facilities Annual Incentive Program
Within Facilities, every employee is responsible for safety. When all employees work together for each other's safety, a safe working culture is created. The Facilities Annual Incentive will aid in the establishment of a safer working culture.
The five-year average for lost-time hours in the USU Facilities Department was calculated to be 3,215 hours per year. Facilities has set a goal to beat the five-year average of lost time hours due to work related injuries by 50%. If this goal is met, the entire Facilities organization will gain the incentive.
